Auditing a Volunteer Bike-Marshal Fault Checklist's Silent Fan-Out Across Three Facilities Queues
A paper from the School of Continuous Improvement audits Spoke Check, a fault checklist for the University's thirty-four self-service bike-repair stands, finding it silently duplicated every filed report into two further Facilities systems for eight weeks before a patch collapsed the duplication within days --- a fix that left the stands' own repair-turnaround figure statistically unchanged.
